englischThe aim of the thesis is the development and evaluation of a model for the design of complex work systems using the example of patient care. The foundation of the thesis is formed by a summary of the current state of the art of complexity sciences related to work design and a system oriented analysis of patient care. The DESPINA model (DESign Principles for Balancing Complex Work Systems IN PAtient Care) was developed in line with the „system engineering” method. DESPINA describes the balancing of four potential conflict areas (planning, performance, control and resource supply) which are coupled with 15 design principles. An evaluation is undertaken for three application examples of this model: the design result („Gesundheitsdaten-Bank eG“), the design process (workshop concept „Fabrica Medica ®”) and the design competence („tool for developers“). The use of the DESPINA model opens up a diverse range of field for research in the area of complex work systems.
Die Arbeit untersucht das Phänomen „Komplexität“ im Arbeitskontext der Patientenversorgung. Es wird ein Modell entwickelt, welches einen Ordnungsrahmen für die Gestaltung von komplexen Arbeitssystemen darstellt. Der Stand der Komplexitätsforschung, eine Analyse der Patientenversorgung und arbeitswissenschaftliche Erkenntnisse bilden die Grundlage. Mit der Methodik des „Systems Engineering“ erfolgt die Entwicklung des DESPINA-Modells (DESign Principles for Balancing Complex Work Systems IN PAtient Care). DESPINA beschreibt ein Ausbalancieren von vier Spannungsfeldern (Planung, Durchführung, Kontrolle, Ressourcenbereitstellung), die mit Gestaltungsprinzipien verknüpft sind. Für drei Praxisbeispiele („Gesundheitsdaten-Bank eG“, Workshop-Konzept „Fabrica Medica ®“, „Werkzeugkasten“) erfolgt eine Evaluation. Es zeigt sich, dass der Versuch Komplexität zu reduzieren neue Komplexität verursacht: Gelungene Komplexitätsbewältigung hängt also von der Akzeptanz der Komplexität ab.
