Gesprächsbegleitung im Advance Care Planning

The implementation of Advance Care Planning (ACP) in residential care for older people places specific demands on qualified facilitators and the stakeholders involved, given the target group and the ACP dialogue process.

In a multi-phase research approach, international findings on this topic were collated and supplemented by the context-specific experiential knowledge of qualified facilitators in Germany. The result is a set of practical competencies for facilitators to design an ACP dialogue process tailored to the target group.

The supplementary recommendations for stakeholders clarify what is required for the successful implementation of ACP in residential care for older people.
